Triage Criteria (May 2016)

DCPNS Triage Guidelines for Initial and Follow-Up Appointments in Nova Scotia Diabetes Centres (2016).

These guidelines are aimed at ensuring timely access for newly diagnosed individuals and more effective follow-up (based on individual need and diabetes status) for ongoing care/education. An additional focus on discharge to the primary care provider (for those that are well managed, showing consistent improvement, and able to self-manage) is intended to foster self-care, keep workloads in Diabetes Centres more manageable, and to allow diabetes educators to focus on time-intensive cases, where needed. 

The FAQ answers common questions from diabetes care providers and providers suggestions on how best to engage, before implementing the discharge criteria.
